The owner/RN moved my sister in May 2023 to IHCS Residential on N.E. side without telling me or my daughter, who was [name removed] POA. The last time her PCP saw her there, in February 2023, he listed "failure to thrive" among her diagnoses. His notes also state Kidney Disease, but the new Nurse Practicioner at IHCS starting in May has no record of [name removed] having Kidney Disease. She was started on Eliquis at Autumn Leaves by someone, and it's a high risk drug for Kidney or Liver patients to take. Still on Eliquis when moved to IHCS, by [removed], while at IHCS, my sister died in ICU…

The owner/RN moved my sister in May 2023 to IHCS Residential on N.E. side without telling me or my daughter, who was [name removed] POA. His notes also state Kidney Disease, but the new Nurse Practicioner at IHCS starting in May has no record of [name removed] having Kidney Disease. She was started on Eliquis at Autumn Leaves by someone, and it's a high risk drug for Kidney or Liver patients to take. Still on Eliquis when moved to IHCS, by [removed], while at IHCS, my sister died in ICU at Baptist Hospital. She hemoragged for 2 days because of the Eliquis before dying. IHCS also told us Suzanne's blood sugar level was 500 when she was moved there, and she never had eyeglasses with her, which she had worn her whole life. This is all documented in doctors progress notes and Texas HHS records.

What are the rooms like? There are not any Companion Suites or One Bedroom Apartments as in the description. They do house men and women together and there are 3 residents to a room and the garage is converted into a room How are your interactions with the staff? I witnessed a high turnover in staff which were not certified nurse’s aides, and I never ever saw any activities while visiting over a 3-year experience. What amenities does the community offer? Smoking area in the backyard.

I can agree the homes are always clean but while visiting on weekends and an occasional weekly visit, I personally never ever saw any activities that the residents engaged in. For example, I stopped by for brief visits on a Friday, Saturday, and Sunday and two residents were in bed all weekend. There were also occasions while visting, where I witnessed the residents being left unattended. Dinners are cooked at one of the other residential homes so the caretakeron duty leaves the residents alone to get the dinner from the other residential home. I was told this is done to cut cost but safety of the residents is not being considered. The owner is an RN she does not reside there abd the rest of the staff are not certified nurses aides or have very little experience in the medical field. The staff now consist of friends and family members.

Wilma provides a great living environment for my sister of 57 years of age. She is happy with the food, bedroom and mostly she loves and respects Wilma. Wilma also takes the clients to doctor appointments. We are so glad to find her. Her R.N. background makes it perfect for diagnosing medical issues that may arise.

This residential facility is not safe. There is a high turnover in the staff. [removed] The patients sit in front of the TV and there are no activities as the owner lists. The staff puts the patients to bed at early hours between 4pm-5pm. [removed] They are lazy and and have no concern for the patients. They pile residents in one room and do not supply hygiene products. Other patients there complain that their items are being used for someone else. This is not the place for your loved one.

They've done a really great job so far, 5s across the board. The woman who owns it is a registered nurse, and I like the fact that they've got someone on hand for care 24/7.

Wilma was pleasant, but I didn't care for the condition of the home. Even the front yard was overgrown, which doesn't give a good first impression.
