My mother died at this facility she had cancer they did not facilitate transport to chemotherapy she had numerous items stolen from her was talked to poorly by staff had bed sores and a pressure ulcer on her foot from poor care and was physically assaulted by a staff member because the staff member was upset with another resident and took it out on my mother-this isnt the place for you or your loved ones they are understaffed and while some likely care thats not the culture i experienced with these people

You will want to buy a new wardrobe and keep the receipts to which you will have them lost or stolen per the few caregivers who visit. their social worker failed to do a proper check in It took six days and check in included no documentation of what to bring, how much, how they protect her property, she spelled the patients name wrong, told me she was going to change my moms health insurance to which I told her no, and to “bring clothes she likes”, which were then stolen because not marked with their name. They put a sign that her poa does laundry. More clothes were stolen. Her poa put up an additional sign and supplied a bag that specifically stated where to put the named clothes and they are passed out to patrons from her closet instead. Excuse is we should write the name multiple times. Much insubordination with the patrons and their checkbooks gouged. I have to hire staff to work with their staff. Social worker refuses to set up consultations with director of nursing so ensure you get a number from the front desk. The director of nursing was able to take preventative actions and collaborate on accuracy of meds since there is no check in process. Director of nursing was so appreciative and thanking. Though it happened 7 days after the patient arrived. I count my lucky stars that the receptionist quickly transferred me to him.

Why is the listed phone number not answered
It is not acceptable for a resident to be sitting with other residents in rotation eating NEAR the nurse station and choke to death. My brother lived here from late December 2022 until he died due to choking on a peanut butter sandwich (with no jelly) in March 2023. My brother liked the food at the facility and the outings. He had lost most of his teeth and any general look at him with no educational degree in any subject should be enough to know he should not have been fed peanut butter. The staff could not do the Heimlich because they said they could not get him up due to his size (220 lbs., 6'1") My brother died at the age of 72 because 1. He was fed something he shouldn't have been fed, 2. He wasn't being observed even though he was being fed in a rotation neat the nurse station, 3. The facility did not do Heimlich and did not have a LifeVac. HHS investigated and found no fault with the facility or in their response.
We’ve been very impressed with Birchwood and its staff, especially Gabby. She has gone above and beyond to help us with paperwork and answer any questions we have. She is kind, patient, and professional. We’ve also been very pleased with the staff in the memory care section. They take good care of the residents and treat them well.
I just want put my review on here about Adon and DON they put people name on schedule that's prn don't check with them see can they work or not just because for they can say they a no call no show then constantly telling stories on employees stated that people told her that i wasn't going to return so they fired me without even calling and for as the new HR lady stating i pick up shift till 3 weeks she need look and see why it's like six of us that couldn't start work till 3 weeks later ask DON she knows why she knows state what in the building they had us wait for 2 weeks to start and they offered bonus for working on the weekend promise us they was going to pay us never received it that's not right and that's not fair like we don't have a family to feed call talk to HR she never return my call so im taking it to next step the company is a good company but its the people you have as your head nursing Department they violate a lot HIPPA rights around there it's not right what they do to they employees my 3 Stars is for the residents they the only ones I'll miss
I can’t say enough great things about this facility! They have been absolutely amazing with my dad and he is not the easiest person to deal with and they have taken so much stress off of me! Amber and Gabby(the nurse) have just been exceptional and I can’t thank them enough for what they have done for my family!
