My grandfather fell at AVIR in Irving on September 1, suffered a head injury, and was transported to Parkland Hospital. What has followed has been extremely frustrating for our family. Despite repeated written requests and a September 8 deadline, I still have not received the accident/incident report. My grandfather fell at AVIR in Irving on September 1, suffered a head injury, and was transported to Parkland Hospital. What has followed has been extremely frustrating for our family. Despite repeated written requests and a September 8 deadline, I still have not received the accident/incident report. A week later, I am still chasing documentation and answers about what happened to my grandfather while he was in their care. Families deserve clear communication, timely responses, and accountability when a loved one is injured. Having to repeatedly follow up for this information during an already stressful situation is unacceptable. I will be filing formal complaints and requesting an outside review. Based on our experience, I cannot recommend this facility.
I would not recommend Avir Irving Nursing Home to anyone who cares about their loved one. My aunt was transferred there after major surgery for a 21-day stay for physical therapy, and the level of care she received was unacceptable. She was not given physical therapy daily as ordered by her doctor, yet staff documented in the system that she was receiving it every day, which is simply not true. I would not recommend Avir Irving Nursing Home to anyone who cares about their loved one. My aunt was transferred there after major surgery for a 21-day stay for physical therapy, and the level of care she received was unacceptable. She was not given physical therapy daily as ordered by her doctor, yet staff documented in the system that she was receiving it every day, which is simply not true. She developed a severe rash from being left in soiled diapers for long periods. When the call button was pressed, it often took hours for anyone to respond. The only time staff seemed attentive was when family members were present and actively complaining. The professionalism of the staff was extremely poor. One tech aide was openly rude and spoke to me with a bad attitude, even telling me to move multiple times. Director Katrice Lee was informed of our concerns several times and repeatedly said she would follow up, but never once did. The social worker, Mariam, was also unhelpful and misleading. She told us my aunt’s oxygen would be ready upon discharge, but when my cousin arrived to pick her up at 6:30 p.m., he was told he had to arrange it himself. Mariam insisted my aunt had to leave due to insurance, even without oxygen, saying she would be fine. Meanwhile, the nurse on duty stated they could not safely release her without it. We also raised concerns with a man who said he was in charge of the facility. He promised to follow up with my uncle, but never did. Basic care was neglected. My aunt went two weeks without a bath—her hair became matted and smelled so bad that I had to wash it myself in her bed. Only after my uncle complained at the front desk did aides finally give her a bath. Additionally, a night shift nurse was overheard speaking negatively about my uncle to staff, unaware he was sitting nearby. The facility itself was also concerning. We saw roaches in the restroom, and it was common to hear other residents crying for help with no response from staff. The only positive experiences we had were with a few male RNs and one male medication tech. Aside from them, the care was consistently poor, unprofessional, and neglectful. It felt like once insurance coverage became an issue, the priority shifted to pushing patients out rather than ensuring safe and proper care. I strongly urge others to look elsewhere when choosing a facility for their loved ones.
My mother was admitted here for rehab after a fall. Not only did she receive poor care, she was injured three times due to negligence. Some days, they didnt get her out of bed. Other days they left her in an uncomfortable wheelchair all day. The only good things I can say is the housekeeping services were good and her room was kept clean and the food was better than an average nursing home. The physical therapy was good, but the progress with PT was not continued in the nursing care. The only good things I can say is the housekeeping services were good and her room was kept clean and the food was better than an average nursing home. The physical therapy was good, but the progress with PT was not continued in the nursing care. The poor nursing care and poor communication and misinformation from the nursing director and social services was unacceptable. I had to take her out and hire home care or she would have died there. Also they advised her to switch from her medicare advantage plan to strait medicare because her plan was no longer paying. This was a huge mistake which caused delays/confusion in getting home health nursing and she is now being billed from this nursing home for the full amount when they told her she would only pay 20% of the remaining balance.
PLease Don't Take Love One's here. The parking is horrible, the place reeks of Urine, the furniture looks like they pick up from thrift store. The staff they pick up from salvation army (they look homeless.) The care is horrible NONE PROFESSIONAL. They left her without oxygen. She and other patients was asking to go to bathroom no never came. They wasn't given her medicine properly. My mom ended up back in the hospital after leaving this place
WARNING: Please do not send your loved ones here. This place is horrible. They have little to no parking for visitors. Their employees are smoking Marijuana on the south side of the building everyday. They're infested with roaches. They sedate patients so they don't have to watch them. They neglect folks and let them sit/lay in their feces to the point of soiling their sheets. They don't empty the urine catheter bags either. They let my mother in law fall out of bed and hit her head. They didn't even notify my wife about her mother falling. They won't brush their teeth or feed them. They seem to always forget that folks need to eat and drink daily. These ppl are not equipped or competent enough to care for a goldfish. Don't leave your loved ones in the care of lazy, negligent, under the influence caretakers.

had a wonderful experience with Toni at Avir Irving. From the moment I spoke with her, she was incredibly patient, knowledgeable, and compassionate. She took the time to answer all of my questions thoroughly and made sure I truly understood every step of the process.
