Beautiful, well kept facility in great location away from the busy life. Hidden gem with beautiful, mature trees around. Very professional, friendly and knowledgeable staff. Amazing administrator who is very passionate about her patients and residents.
Like: Price and location are the two main factors. 1. Facility maintenance is lacking and in dire need of updating. 2. Communication/ Internet signal strength is a chronic problem as well as TV programming service. 3. Food selection and quality is mediocre
If I could give a negative rating I definitely would. This place is truly atrocious on so many different levels. The safety aspect for it to be considered a secured memory care facility is a complete joke. There were multiple times I went to visit my loved one and I would not see a staff member in the lobby/open areas or upstairs in or around the nurses station. There were times I was in with my loved one for multiple hours and not one tech or nurse would walk by let alone come in and check one her like they were supposed to be doing since they were a fall risk memory care patient. There were…

There were multiple times my loved one was not offered/fed meals, and the meals they did feed they did not follow her agreed procedures in which caused either non eating or chocking due to lack of supervision. The management team does not communicate with the families and/or other staff and they definitely don’t actively supervise their staff. They also do not take residents to scheduled appointments. This facility also has plumbing issues that are not taken care of and overall cleanliness is questionable. The only good thing I can say about this facility is that the physical therapist is amazing and does amazing work with the residents/patients. I am truly disappointed at the level of care (well lack there of) during this hard time for our loved one and our family.

Majority of staff is friendly and respectful. Quite a few activities but not much participation, though this is no fault of Cottonwood.
Excellent staff. Food and cleanliness is a priority. CMS rating is a 5 Star for quality of care.
I am a former resident of Cottonwood Creek Assisted Living. My review is based on my experience in Assisted Living and not Skilled Nursing. Many of the reviews posted are for Skilled Nursing Residents. This facility does have some serious problems. I do have great respect for the aides and med techs. The facility has several maintenance needs that have gone unattended although residents have made complaints. The communication between upper staff and residents is non existent. Occasionally meetings are set up to discuss emergency situations but the day to day questions and needs repeatedly asked are never answered satisfactorily. Assisted Living had one or two activities planned. You are able to go upstairs and participate in activities for skilled nursing. That was enjoyable and I can’t express enough the wonderful Activities Department, Darryl and Patsy and their dedication to the residents. They were amazing! However, there are no outside activities provided and most residents needed a trip at least to Walmart for personal items. It is also good to get out of the building for a change of scenery. Some residents never see anything outside of the building except the parking lot. There were bug issues and my apartment, even after being sprayed, had them crawling everywhere. Some residents had none which I greatly envied. Many visiting don’t understand that this facility has institutionalized food. Everything is processed and out of a package or can and everyone upstairs and down receives the same food. There are no special diets available. They do have an options menu. The food is lukewarm or cold by the time it reaches the room and very rarely is it any good, sometimes downright gross! The menus need a big overhaul. There was a drug and alcohol problem with some residents but I believe most of it has been resolved. There is one review recently submitted by the building administrator and I think it is not appropriate for her as the top executive of this facility to give an excellent review as she is currently employed there and that was not clearly stated. There was a feeling of uncertainty about the financial state of the facility among the residents. Things needed were never received, repaired or it was delayed for months and we were told it was due to finances. In the beginning there were parties at every holiday and by the end of my leave there were none due to budget restraints. They did manage to maintain the party for Residents of the Month birthdays. The State was always coming and made several unannounced visits which made the staff scramble and the residents nervous wondering what was going on now. The facility has much potential if they would upkeep the property and not wait until it is mandated or fined. They need to listen to the residents and follow through on the promises it makes to them. If you are looking for an inexpensive place to live or place a loved one, this is the place. Be warned that a tour is often misleading and you usually get exactly what you pay for. I have much love for the residents of Cottonwood Creek and I pray that at some point their needs will be met other than a roof over their heads and 3 meals a day because other needs not being met are just as important!
