This is a review for the long term care side of this nursing home. The short-term Rehabilitation and Physical Therapy side is not mentioned in this review. My mother lived at this facility for two years. While daytime care was generally adequate, nights and weekends were consistently poor and neglectful. This is a review for the long term care side of this nursing home. The short-term Rehabilitation and Physical Therapy side is not mentioned in this review. My mother lived at this facility for two years. While daytime care was generally adequate, nights and weekends were consistently poor and neglectful. For over a year, I raised concerns specifically about evening and weekend care, yet these issues went unaddressed and showed no meaningful improvement. There were multiple instances—especially on weekends—where my mother was left unchanged for extended periods. She often had to call me so I could contact staff to get basic care addressed. My mother frequently complained that her call light would go unanswered, leaving her unassisted in her room despite being bedbound. On several occasions when I came to help her myself, I observed CNAs at the nurse’s station playing on their phones instead of responding to residents, or no one at the nurses station at all for extended periods of time. The food quality and variety were also concerning, with the same meals repeated several days in a row. Communication was one of the biggest failures. In two years, I was never able to speak directly with her physician despite multiple requests. I was never even given the doctor’s name, and staff could not provide clear answers. This was especially troubling since I needed to be involved in her care. There were repeated hospitalizations that appeared preventable with proper monitoring and timely care. She was sent to the hospital multiple times for infections, including severe UTIs and pneumonia. In one case, an ER physician described her UTI as the worst he had seen in his career. In 2026 alone, she was hospitalized four times in four months. The facility was also reluctant to arrange hospital transport when needed. At times, my mother had to call 911 herself, or I had to intervene and insist on transport. During these critical situations, I was not kept informed of lab or X-ray results. When I raised concerns and asked what steps would be taken to prevent future incidents, I was told that if I was unhappy, I could move her. After that, repeated attempts to contact leadership—including the Director of Nursing and administrator—went unanswered. We ultimately moved her. No one from the facility has followed up to ask why. The building may look nice, but the level of care, communication, and responsiveness was deeply concerning. I cannot recommend this facility.
The staff was so caring and very helpful The front desk to the gym everyone was outstanding. If I needed anything the nurses or anyone that would pass through was very helping. I recommend any and everyone to try forum Parkway rehabilitation facility
I attended a Thanksgiving lunch for families. The staff amazed me at their attentiveness! I saw residence struggling to reach their food and just as quickly, a cna or other staff member was helping them. The activity director LadyBug was full of energy and made a point to interact with the families. The residences seemed to smile and enjoy interacting with every staff member they encountered and gave me a very good feeling about the whole facility!
My time as a long term resident has been an easy transition so far. The staff here at the facility have made my life easier and more eventful. The activities director has even implemented me into having my own wheelchair workout class in the morning. The receptionist Ashlynn and Mitchell are exceptional and extremely kind. I would recommend Forum Parkway for your loved ones.
The staff was amazing. Came for rehab but ended up having to have hospice come in and graduate my family member on to heaven. Team worked great together and we had the best experience ever here!! High recommend
The level of quality / dignity service has declined drastically in this location. We had My 93 year old mother stay here twice. Once for rehab and another for respite care for 7 days. I could not believe that the level of service had changed so drastically. One could say they were either short staffed or just stopped providing the same quality they use to. I saw plenty of staff just not the caring side this time around. On top of all that, we are still waiting on a refund that the administrator told me was is in the mail. That’s going on 3 months now. We had to get hospice involved as they had paid as well. This tells me nothing is urgent from the staff to the administration of this facility. Totally different experience from our first time. Very unprofessional and sad as our elderly deserve quality treatment with dignity.
My experience with Forum Parkway Health & Rehabilitation has been extremely disappointing and deeply concerning. This facility is supposed to provide care, comfort, and support for residents, but what I experienced was the exact opposite. The administrator, Dylan Gadberry, is very dismissive. It is clear that he does not truly care about the concerns of families or the well-being of the residents. Instead of addressing issues seriously, he brushes them off, leaving families feeling ignored and unsupported. The speech therapist, Katherine, is extremely rude and unprofessional. She made it clear she did not want to be held accountable for her behavior, including being overheard in the hallway discussing patient status — which is a serious HIPAA violation. For a facility entrusted with residents’ privacy, this type of conduct is completely unacceptable. In terms of actual medical care, my mother only saw a doctor 2 out of the 14 days she was in the center. She received physical therapy on just 7 of those 14 days — meaning she spent half her stay with no therapy at all. For a rehabilitation center, this lack of consistent medical oversight and therapy is inexcusable. Cleanliness is also a major issue. The very first room my mother was placed in had bugs, which should never happen in a healthcare facility. On top of that, both restrooms smelled strongly of urine, showing a lack of basic sanitation and proper upkeep. Staff response times are extremely slow, and the facility seems severely understaffed. When concerns are raised, they are met with minimal effort or superficial fixes rather than real solutions. Families trust facilities like this to provide safe, quality care for their loved ones — but Forum Parkway Health & Rehabilitation has repeatedly failed to meet even the most basic expectations. This center needs serious improvement in leadership, staff professionalism, medical oversight, cleanliness, and patient care. Until these issues are addressed, I cannot recommend Forum Parkway Health & Rehabilitation to anyone seeking a safe, supportive environment for their loved one.
The facility is nice and well kept. My 82 year old dad was at this facility approximately 3 months. My family didn't have many facilities to choose from since my dad was a trach patient. We chose this facility due to location, and it was well kept. The staff did an okay job. Some that stood out and went above and beyond for my dad were: CNAs (Torina and Pride). They do awesome work! All CNAs should be trained by these two individuals. Torina was very patient with my dad and took her time ensuring that he was well tajen care of EVERY single time she had him as her patient.The respiratory therapist, Shauna, was a God send for our family🙏She worked with my dad daily and was always patient and polite. Lobrado did well too. Jessica over wound care did good ensuring my dad didn't have any bed sores, etc. She was always nice and patient with my dad. She also answered any questions that I had. Nurse Bhawana, Elizabeth, and Punam were good.The night (10pm-6am) and weekend staff didn't check on my dad much. This was a concern for me. I was very please with the following therapists: Anthony, Shoba, Natalie, and Slone. They all did an excellent job working with my dad! I do NOT recommend having Shailah or Megan as your loved ones therapists. They both rushed through my dad's sessions and didn't put forth much effort. They weren't patient with him either being that he is 82 years old. Shailah was rude towards me, not professional, and was irritated whenever I asked questions. I was surprised when I found out she is the Asst. Director of Rehab, because her communication was poor and she doesn't display good customer service at all! Megan was nice but lazy and didn't do well with my dad and his therapy sessions. Putting a camera in my dad's room helped me and my family keep an eye on my dad. I recommend you do this. The camera also allowed us to see how the staff treated my dad when we weren't there. I was at the facility 5 days a week/ 6-7 hours per day, so I was able to see a LOT in person. Joe Gomez was nice from day one. He ensured the room was stocked with supplies and often stopped by just to say hello and see how my dad was doing. That meant a lot to me. Upper management did not communicate well with my family. They walked pass my dad's room daily and never stopped by to see how he was doing. None of them seemed to care. They're customer service and response time needs improvement too. My overall experience with this facility was just okay. I had many stressful days worried about how my dad would be treated. I am THANKFUL to those that I mentioned and appreciate their professionalism, caring ways, and work ethic. I hope this review helps.
