My dad recently passed away. I am posting this to warn other families about the severe lack of medical oversight and professional boundaries I witnessed during his final days. 1. Medical Neglect: My father was suffering from pneumonia, a diagnosis I was not informed of until 24 hours before his passing. When I alerted the nurse, Renee, that my father was shaking uncontrollably (and provided video evidence), she dismissed my concerns, stating "he always does that." Medical Neglect: My father was suffering from pneumonia, a diagnosis I was not informed of until 24 hours before his passing. When I alerted the nurse, Renee, that my father was shaking uncontrollably (and provided video evidence), she dismissed my concerns, stating "he always does that." When I corrected her and said, "No, he doesn't," she got sassy and snapped, "Well, we work with him every day." I told her, "No, not like this, he's not okay." When she finally realized he could not squeeze her hand or respond, she left the room without taking further action or calling for help. My dada passed away the following day. 2. Infection Control Breaches: Nurses Renee and Christina frequently allowed their children to run unsupervised through the facility. These children were observed entering isolation rooms where PPE (gloves and gowns) is strictly required for staff and visitors. This is a massive breach of safety and infection protocols. Look past the "helpful" facade the staff puts on. Nurses Renee and Christina are experts at appearing attentive when family is present, but their actual care is lazy, deceptive, and dangerous.
This location has really gotten so much better and fantastic staff. Roslyn is such a pleasure to talk with.
Health Department needs to check this facility. It is not clean and it smells horrible. The baseboards and floors in the rooms are filthy. I literally wanted to get a mop and broom and start cleaning. The smell is nauseating. Please Go check the inside end wing by the apartment complex . Very unhealthy!! Don’t just take my word for it go for a visit. This facility should be spotless and have a fresh clean scent. I visited on a Sunday.

Amazing and friendly staff! The place is clean and doesn’t have any odors. The place is welcoming and inviting. Highly recommend!
Not enough help to properly meet all residents needs
The facility rarely answers the phone. The doctor had to call ME to ask Paradigm to call them for my relative’s bloodwork. My sister & mother have not had their phone calls answered in the last two weeks. Other patients’ families have told me the same thing. It just rings & rings & rings. Also, no communication between the staff. I told the nurse I was taking my relative to the doctor but they failed to let other employees know & they transferred my relative to the doctor instead of me.
This facility doesn’t even deserve one star! Constant personnel turnover up to and including administration. The place reeks of urine and feces the minute you open the front door. I witnessed the residents being served hamburgers that had burned buns and meat. Oranges that were quartered were served to residents who could not remove the peelings due to physical challenges. Personnel were indifferent to the needs of the residents! Paradigm ownership hasn’t positively changed anything at the former “Gardenvilla” facility. You should be investigated for cruel and inhumane treatment of the residents!
