DO NOT SEND YOUR LOVED ONE HERE. My mother was transferred to Parklane West Healthcare Center for rehabilitation after suffering a heart attack. DO NOT SEND YOUR LOVED ONE HERE. My mother was transferred to Parklane West Healthcare Center for rehabilitation after suffering a heart attack. She had been identified at the hospital as a high fall risk, and I expected that information to be communicated and acted upon when she arrived here. When I arrived today, I was extremely concerned by what I saw. The facility had a strong urine odor, and I observed residents wandering the halls and sitting in wheelchairs without what appeared to be adequate assistance. Within the first hour of my mother being there, she needed help getting to the restroom. She did not receive the assistance she needed, and she fell. At the hospital she had a clear high-fall-risk designation, yet I did not see any fall-risk warning posted on her door or evidence that appropriate precautions had been put in place. This is completely unacceptable. My mother had just suffered a heart attack and was sent there because she needed rehabilitation and care—not to be left vulnerable to another injury. I am extremely disappointed and concerned about the care and safety of the residents at this facility. Families need to know what they are walking into before trusting this facility with their loved ones.

“Unfortunately, my experience was disappointing. The front desk staff spent a noticeable amount of time gossiping and talking among themselves instead of focusing on visitors. It made me feel like customer service wasn’t a priority, and the atmosphere seemed unprofessional. I understand everyone has conversations at work, but visitors should feel welcomed and attended to. I hope management addresses this so future visitors have a better experience.”

This was the third time I have been there as a patient. So much love everywhere I went. I will always go back . Cheryl Ramsey

I was very satisfied with the attention given by the nursing staff and crew they were nice and courteous to my needs and comfort

They gave excellent care, especially CNA’s Rocky and Beverly on the third floor. I was in room 307, and they both went above and beyond in their care of me. Rocky and Beverly both treated me with sensitivity and dignity. I am forever grateful.

My mom was there 12 days. Her sweater disappeared. Money was taken from her wallet and they serve sausage.Eggs every day for breakfast

Staff was very good and ot and pt very good .

Blatant Negligence. Please don't send your loved one here. While here, my family member was left unmedicated and developed a severe infection. We voiced our concerns to the DON Candace Bennett, Assistant DON and many nurses to include a short female nurse Joe. My grandmother was trashing with discomfort and anxiety for 6 days and nothing was done by the staff to help her. They all ignored her. On the 6th day we called 911 and had her removed for her safety. We were wrong to trust this staff and facility. She left this place with a severe infection, kidney damage from dehydration and a bleeding bald spot from thrashing.
