Ask about this communityThe Broadmoor at Creekside Park in The Woodlands, Texas is a 112‑bed skilled nursing community providing 24‑hour nursing and a range of clinical programs, including post‑surgical, cardiac, pulmonary, stroke and transitional care. Rehabilitation offerings include physical, occupational and speech therapies, outpatient rehab, restorative nursing and specialized clinical services such as wound management, enteral and parenteral care, intravenous therapy, tracheotomy care and telemedicine support.
Accommodations include private and semi‑private rooms, some with private baths and in‑room kitchenettes, alongside communal areas such as a library, chapel, salon/barber, interior courtyard, outdoor patio and an on‑site café. The community also lists pharmacy services seven days a week, laundry and housekeeping, resident transportation for appointments, activity programming, discharge planning and social services to assist residents during transitions.

I had a wonderful experience visiting this skilled nursing facility. The building was clean, the residents appeared well cared for, and the staff were welcoming and attentive. What stood out most was my interaction with Alexus in Admissions. She was extremely helpful throughout the entire visit and made sure I had all the information I needed. Alexus was professional, approachable, and took the time to answer my questions in a way that was easy to understand. The tour was thorough, and she did an excellent job explaining the programs, services, and level of care available.

My dad was sent here after back surgery because he needed temporary help with basic things like getting to the restroom, bathing, and recovering safely while doing physical therapy. Before surgery, he was fully mobile, independent, and did not wear diapers. We trusted this facility to provide basic care while he healed, and unfortunately that did not happen. The biggest issue was repeated neglect when he needed help getting to the restroom. On one occasion, a staff member actually told him to “just poop in his diaper” because they couldn’t help him. That is humiliating and unacceptable for anyone, especially someone recovering from major surgery. Another day, my dad asked for help using the restroom in the morning and no one came. He yelled for help so much that he almost lost his voice. Hours went by with no assistance. I finally had to call the facility myself around 4:30 PM and complain before someone finally responded. No patient should be left sitting for hours needing to use the restroom while repeatedly asking for help. The worst incident happened when he became so desperate to use the restroom that he tried getting up on his own because no one was responding to him. He ended up having a major accident, reinjured his surgical area, and is now in significantly more pain than he should be during recovery. As a family member, it’s heartbreaking and infuriating to watch. I understand healthcare workers are busy and understaffed, but basic human care and dignity should never be ignored. Families place enormous trust in these facilities when their loved ones are vulnerable. Based on our experience, I would strongly encourage others to ask very hard questions about staffing, response times, and patient care before placing a loved one here.
One of the worst experiences I have had. Nursing Aids would walk in my room to provide care and be talking on their cell phones via their Air Pods. One of the Aids actually walked in my room talking on her cell by holding her cell phone to her left ear with her shoulder. So not only rude but how can good patient care be given while talking on their cell phones. I couldn’t stand or walk without assistance. One Aid was so mad that she had to come and change me. She drug the wheelchair over to my bed and said “Mr. Woods here i is a the wheelchair for you to get up and get yourself to the bathroom!” I said that I am unable to walk or stand without assistance.” Her reply was “I only work part time and PT didn’t tell me.” Obviously there needs to be communication between the shifts. A counselor/ therapist came in and asked me if she could ask me a few questions and I agreed. About 10 minutes into the session she said “you aren’t a woman Woods are you?” I said “no I haven’t had gender reassignment surgery.” She walked next door and found the woman Woods she said “I’ve been doing that a lot lately.”’ I thought WOW that is very concerning. That same counselor came back a few days later and asked if she could finish asking me questions, I told her to leave. I have a lot more to say but may run out of space. The PT and OT were so kind and all of them were encouraging all of us even though they weren’t working with each of us. They offered to help each other, they displayed great teamwork and a very cohesive team in comparison to the nursing staff who truly do not understand by working as a cohesive team that good patient care can be provided. Now “T” and Gemayel had only recently been hired and placed in charge of the nursing staff. They have made some positive changes, but they have a along road to travel down to having a successful staff. I truly wish “T” and Gemayel good luck. The Housekeeping staff was so friendly, helpful and worked well as a team by offering to help each other. As you can see I could only give this facility a “2”. If you place a family member or friend at this facility please check on them on a regular basis. You may need to be their advocate while at this facility. The reason that I only addressed the woman that the “counselor” confused me with.as “Woman Woods” because I didn’t have her permission to list her name.
The most irresponsible “care” ever! Do not send your loved ones there. They do nothing! Patients are screaming for help and noone pays attention. When we called the manager they said she was in a call all day and could not come to the phone.
I would not recommend this facility to anyone who cares about their loved ones. The level of neglect I witnessed was deeply concerning. My mother was left in unsanitary and unacceptable conditions, which should never happen in a care setting. What was even more disappointing was the lack of accountability from leadership. Director of Nursing Turania Jones and the staff did not take responsibility for these issues or show urgency in addressing them. Families trust facilities like this to provide safe, compassionate care, and unfortunately, that standard was not met.
