I was a patient in the beginning of 2024 and it was the worst experience I have ever had. They have medical staff that would not be hired at any hospital or clinics because they are not up to date. The nurses assistants are not much better because they don't care about each other much less a patient they don't know. The rehab department has one person with any safe experience while the others just seem like they think torturing people is a form of rehab. The nurses assistants are not much better because they don't care about each other much less a patient they don't know. The rehab department has one person with any safe experience while the others just seem like they think torturing people is a form of rehab. They have many things wrong with them and I think they are dangerous for some patients that think they are going there for care. There were only three staff workers that I feel were really there to help patients. I have so much more to say if someone wants to hear how they mistreat patients.
If I could 0 stars they told me to give them a chance to treat my mom 1st week was there didn’t see anything called nurse to hey my mom clean up she was choking respetorory didn’t show up till 1 hr later mom is back to ICU if you want your love one to die take them there
This place is the best nursing home on the southeast side of San Antonio, it always clean. My grandma loves it! Friendly staff, attentive.
If a facility could receive zero stars, this would be it. This facility is clearly understaffed. A patient was released before even completing two full days because they did not have enough staff to provide care. That alone is alarming. I was also told there have been five resident falls, which raises serious concerns about supervision and staff training. A nursing home should prioritize patient safety, and based on this experience, I would not trust this facility to provide adequate care.
My father was just recently sent there from the ICU, not even a week later my father was ruched back to the ICU do to him not getting proper care and he had caught pneumonia and was rushed at 6am but THEY NEVER NOTIFIED ME THAT HE WAS RUSHED TO THE HOSPITAL. I was so scared when I had arrived to visit my father because he wasn’t in his room and all of this stuff was still there. And the nurses just acted like it was normal no sorry, no apologies and no answer to why I was not notified. During his little time there they never moved him side to side so he wouldn’t get bed sores, they never cleaned him properly and the room was horrible they had stuff in his room that he didn't even need. they threw stuff in there like if his room was a storage unit. and Don't get me started on his nurses were rude and when i asked them about anything or to do something, they acted like it was a big task for me asking them to clean and change him, like it wasn't their job. I personally would not recommend this facility to anyone especially if you think that your loved one is supposed to be getting better with their current situation but it just got worse.

This facility is absolutely unacceptable. The staff is extremely inattentive—often seen sitting around on their phones, laughing, and not paying attention to the residents. My grandmother was there for just 22 days, and each time we visited, we found her in distress—lying sideways with her feet hanging off the bed. Despite repeatedly asking staff to change her or provide basic care items like washcloths, nothing was done. They would say, “Okay, we’ll take care of it,” but hours later, her condition would remain unchanged. It felt like they kept her sedated just to avoid dealing with her. On our last visit, we found out her blood sugar was at a dangerously high level—548. When we asked why nothing was being done, the response was simply, “She’s fine, we gave her insulin.” But she was unresponsive, had a UTI, and still received no fluids or antibiotics. They were clearly neglecting her medical needs. We had to request that she be sent to the emergency room. By the time she got there, her blood pressure had dropped dangerously low. She’s now in the ICU, diagnosed with kidney failure, a UTI, and high potassium levels—multiple life-threatening conditions that were completely ignored by this facility. The doctors told us that had we not brought her in when we did, she might not have survived. I don't recommend this place. Most of the staff appear lazy, inattentive, and completely unconcerned with the wellbeing of their patients. Please do your research before trusting a facility with the care of your loved ones. This place nearly cost my grandmother her life.
