Liz Wilkerson (Therapist) was an incredible help during my stay here. She showed great compassion and understanding while supporting me on my healing journey. My father is being discharged from hospital and has a referral into facility since last Friday 6/5. Here it is Tuesday 6/9 and we thought today would be the day we were transferred, it being a long process we figured it would take a few days. So the insurance approved the stay today, at Tuscany Village. So we call Tuscany village and they say they haven’t gotten the approval yet. Our case manager at the hospital calls and finally gets the story straight and Tuscany Village does not have a bed!!! They knew this all day and informed our case worker of it at 2:45pm. My father is being discharged from hospital and has a referral into facility since last Friday 6/5. Here it is Tuesday 6/9 and we thought today would be the day we were transferred, it being a long process we figured it would take a few days. So the insurance approved the stay today, at Tuscany Village. So we call Tuscany village and they say they haven’t gotten the approval yet. Our case manager at the hospital calls and finally gets the story straight and Tuscany Village does not have a bed!!! They knew this all day and informed our case worker of it at 2:45pm. This is completely unacceptable; dishonest and very greedy. I understand they did not want to lose the business but there is a very real person on the other end of this situation and they have pushed back his rehab by at least another 48 hours, due to having to get another approval. Please find other options for your loved ones, these folks were dishonest with us before we even stepped foot in the building. I am exploring options on how to report to Medicare. This should not happen to anyone already going through a health crisis.

DO NOT SEND ANYONE YOU CARE ABOUT HERE. My mother died because of their neglect. The first two weeks my mother was at this facility the staff and care were wonderful. After that, her care quickly began to decline. When we started asking questions, staff always had explanations, but we could clearly see that my mother was no longer receiving the same level of care she had initially received. One evening, my sister went to visit her and found her in a comatose state, only moaning. The day before she had been talking and laughing with us like normal. A CNA told my sister that she had been concerned about my mother all day and had repeatedly brought her concerns to the nurses, but they ignored her. She then reported it to the head nurse, who still did not check on my mother. My sister had to call 911 herself because no staff member stepped in to help. At the hospital, we were told my mother was severely dehydrated, had a UTI and the PICC line in her arm had not been placed correctly, causing a serious infection. Two days later, my mother was placed on hospice care and later past away. When I contacted Tuscany Village to discuss what happened, my concerns were dismissed, and I was told they had done everything correctly. The day after my mother was taken to the hospital, my husband went to remove her belongings from her room, and not a single staff member questioned why he was there or asked about my mothers condition. Unless you are able to stay with your loved one 24/7, I strongly recommend staying away from this facility.

The discharging hospital provided five recommended skilled nursing facilities, and Tuscany Village was one of them. Before making a decision, I spent considerable time researching CMS ratings and guidelines. Unfortunately, Tuscany ranked as the best among the five options, which in hindsight was a very low standard to meet. I regret not finding the rehab center in Baytown sooner — the facility that ultimately made a meaningful difference in my husband’s recovery. Despite the excellent efforts of that team and the physical progress they helped him regain, the emotional and psychological toll from his stay at Tuscany Village had already taken a significant toll. Before writing this review, I wanted time to process the experience and evaluate my husband’s condition after leaving Tuscany Village. Based on his recovery since discharge, I can honestly say that Tuscany did not deliver the level of care we expected and likely delayed his recovery. When my husband left the hospital, he was medically stable and physically improving. However, within the first week at Tuscany Village, there was a noticeable cognitive and physical decline. His memory worsened, his mood shifted dramatically from hopeful to what I would describe as depression, and he began exhibiting behaviors and tendencies that had not been present during his hospitalization. There were repeated inconsistencies in staff attentiveness. Meals were missed, often followed by questionable explanations. There was no meaningful walker training, no bedside commode provided when needed, and physical and occupational therapy appeared minimal and inconsistent. One physical therapist even stated that they “get paid whether or not he does the exercises,” which was both shocking and deeply discouraging to hear as a family member advocating for his recovery. It became clear that patient concerns were often treated as resistance or “pushback” rather than legitimate feedback. Emotional wellbeing and the psychological adjustment to physical limitations appeared to receive little to no consideration. Medical oversight also seemed limited. The nurse practitioner often appeared more focused on appearances and administration than meaningful patient engagement or support. The facility itself felt dark, dated, and depressing. Most patients remained isolated in dimly lit rooms for much of the day. Despite what the website suggests, social engagement opportunities appeared minimal. I visited nearly every day and never once observed patients dining together in the dining room. Meals were also far from what I would consider heart-healthy or nutritionally supportive for an elderly population focused on recovery and rehabilitation. Additionally, personal belongings were not properly safeguarded. My husband’s phone charger was stolen, his glasses went missing, and soiled clothing was frequently mishandled without concern. To be fair, there were a few staff members who genuinely tried to help and did the best they could within the limitations of the system. However, as the administrator candidly told me, “they are a business after all.” Unfortunately, that statement explained much of our experience. I would not recommend Tuscany Village. After spending two weeks at a rehab facility in Baytown followed by two weeks at home, my husband has improved tremendously. The transition home was difficult at first, but with patience, persistence, familiar surroundings, healthy meals, and simply listening to his concerns, he has made remarkable progress. For families who must rely on skilled nursing facilities, I strongly recommend visiting in person, asking detailed questions, taking notes, observing the environment carefully, and reading reviews thoroughly. CMS ratings can be helpful, but remember they often reflect only whether a facility meets minimum standards — not whether it provides compassionate, effective care. At the end of the day, these facilities are businesses, and the quality of care often depends heavily on the individual staff members assigned to your loved one.

The Administrative, Nursing, Cna , Rehabilitation, Doctors and the Therapist staff gave my husband great care during his two week stay there recovering from a recent surgical procedure. I was kept informed with his daily recovery progress and medication intake. The room and bathroom were cleaned daily. I would recommend Tuscany Village for anyone needing recovery treatment.

I have to give Manny a five star review! Manny is the real deal. He is the activities director. Manny takes the time for all his patients and residents. He is compassionate and respectful. He is fun and charismatic. Manny is creative and follows through what he says he is going to do. I am a Hospice Volunteer Coordinator and I had the honor of doing a stuffed animal pet adoption with Tuscany Village a few times. And every time, it was so beautiful to see these patients and residents smile! Manny and his team really genuinely care ! Thank you for inviting me back. It was so special.

Elizabeth is the most valuable CNA at this facility. She comes in with a smile, willing to help the patient (no request is unreasonable) for Elizabeth. When she says she will be back, she does. She outshines all others. Love her.
