If ZERO STARS were an option, this would be the place. Terribly managed facility, dangerously under staffed, and inattentive. Patient dignity and privacy not respected. POOR/NO COMMUNICATION. Patient care skills severely lacking. Would never recommend this facility!
Can not get in touch with your loved ones after hours. The main number is not answered and does not transfer to the nurse's stations, so you and your loved one are totally isolated without communication. The cafeteria is empty. They do not encourage socialization. I have seen no social activities. It looks nice, but lots of problems.
This place is just awful. The only good thing is some of the staff is nice. The residents rooms are dirty and old. There are stains everywhere on the carpets in the living quarters and hallways, some of the worst smells I have ever experienced while walking around the complex. I had to answer the front door for medical deliveries as the staff wasn't paying attention even though they were all at the work station. I witnessed the nurses not following proper medical procedures. There's no kind of security and they give out the door codes to anyone and everyone. The food, if you can call it that is absolutely atrocious. The director of nursing there is useless and will just argue with you. One of the staff members told me not to contact a family member and discuss what goes on there! They will talk about how they are under new management, but Avir apparently doesn't care about what goes on at their newly acquired facility. This place should be shut down as it's actively a danger to residents.

This is NOT a good place to put your loved ones who need 24 hour care. Numerous times they have left my loved one in their pee and poop FOR HOURS. I asked a month ago that he gets shaved and I FINALLY found a caring CNA to shave him. They don’t get him up for meals and they don’t shower him regularly. They have labeled him a troublemaker for speaking up for himself. THIS PLACE NEEDS A MAJOR OVERHAUL IN MANAGEMENT AND ALL STAFF.
This place is Hell. First night was a disaster grandma was admitted into facility at 6pm. She wasn't given any pain medication for pain and lost control of her breathing because of it. Shes 83 years old. She was recovering from sepsis and pneumonia. She has chronic diarreah and They did not change her bedding or gown. She was sitting in diarrhea for hours. When I pleaded with the nurse who was really rude and dismissive from the first hello she did not want to be there and it showed. There was a tenant on the floor and hit her head and she's laying on her back for about 30mins before she noticed "i told her btw" and she left her on the floor after sitting her up on the floor and continued her round on the floor. My grandma just needed Tylenol since her norco wasn't gonna come in till 12am per the 1st shift. 2nd shift for the overnight 6pm was torture. I hate this place for the fact this ONE NURSE refused to help my grandma because she was "not in the system" but yet we were told she was. I NEED to speak with the head director
My daughter has been a resident of this facility for the past 2 years. She is doing very well and enjoys her independence. The location is ideal ; the staff has taken good care of her needs. She enjoys being able to choose her meals. She is pleased with her room.
