I’ve seen many communities and this is one of the worse. Do yourself a favor and talk to staff not management and listen to your surroundings it will tell you a lot more then management will they just wanna take you money.
Angela, Vanessa and their staff have one of the hardest jobs in the world and it’s comforting to know they are caring for my sister. They go above and beyond allowing her to call when she needs to reach out, printing emails and pictures. Not being able to visit often is difficult enough, knowing I can reach out to staff and they will respond means a lot during this difficult time.
Autumn leaves is a wonderful place, the staff is so nice. They treat my grandmother very well she loves it.

I have had great experiences with Autumn Leaves! Angela and her staff are always a pleasure to work with. They are so attentive to all of the residents and their individual needs!
My mom spent 3 1/2 years at Autumn Leaves. When she first moved in, she was still walking and they kept her engaged with various activities. Although we experienced a few issues, we visited every day and were able to stay on top of them. Over time, however, she started to have more frequent falls and eventually ended up in a geri-chair. On November 16th, when we went to visit her for lunch as usual, we noticed that the air cushion she was sitting on was both wet and had a strong odor. We asked the staff to help pick her up so we could remove the cushion, and to our horror, we discovered it was full of maggots. This is absolutely unacceptable! The staff explained that it was the night shift's responsibility to clean the chairs, but it was clear no one was overseeing this to ensure it was being done properly. Unfortunately, Autumn Leaves has gone through multiple staff changes, and the quality of care has deteriorated. While there are still a few compassionate workers, the overall care is lacking. After much consideration, we decided to remove my mom from there and bring her home for her final days, where she could be surrounded by family in a more caring and comfortable environment. If you are considering leaving a loved one here, I strongly advise staying vigilant and regularly checking on their well-being. After my mom had unexplained bruises, we decided to install a camera in her room to keep a closer eye on things. Do not trust this facility to take care of your loved on! They do not care. They try to say the right things but leadership fails to make sure the workers follow through. Poorly ran and owner could careless. We brought mom home. We had to because she was sitting on maggots because of this she declined rapidly and passed away within two weeks.
Trust your gut, and don't let the smooth taste fool you! This facility is not a safe place to leave someone with memory issues. So many unexplained issues were noted within the first 30 days. To list a few, knee abrasion that turned into cellulitis with no explanation for cause. Multiple bruises on elbow and upper arms were noted by the ER. Medications placed in offices are then offered to residents. Meds left in the residents room. Pt. transported without jacket to Dr. appt. No access to water 24/7. No tissue. No milk. Dirty laundry for weeks, missing clothes, improperly trained staff from top down. No processes or protocols are in place to ensure resident safety. Rough handling of residents was witnessed. Requested a conference call with leadership, which included Ciron N. (Regional Director) to voice concerns and decline in residents' health. They were very defensive and inconsistent when reporting events and safety protocols. As a result, the Regional Director called family, yelling at them, stating,"the Resident will need to get out of his facility tonight" (during an ice storm). They even called 911 and lied on the resident, stating he was hitting people. Luckily, they assessed the situation and did not find any acute reason to transfer the resident to the ER. Since that didn't work, they called and emailed and told family that he could no longer reside there because the physician group with Dr. DOLLIHITE, accused of malpractice, discharged him from their services without notifying family. The list can go on. If you need any specifics, feel free to reach out. Unfortunately, the benefit of the doubt doesn't benefit noone in the healthcare industry. It just leads to poor pt outcomes and malicious business practices. Update: 2/7/25 The facility has not responded to the family or issued a refund since the patient was discharged as a result of the care received
If you’re looking for a beautiful community that feels just like home , with amazing people it would be Autumn Leaves of Cityview… My tour with the Sales Marketer Darlene was astounding . Darlene, was very helpful and I also have to say the Executive Director Angela is very nice and answered any questions I had.. if you haven’t go book your tour ..
