The 125-bed community provides 24-hour licensed nursing oversight, a 20-bed ventilator and tracheostomy unit with respiratory therapist coverage, and on-site physical, occupational and speech therapy delivered in a 3,000‑square‑foot gym.
Resident rooms include private and companion suites, many renovated with roll-in showers, individual climate controls, smart televisions and USB charging. Daily life centers on chef-directed meals with dietitian oversight, scheduled therapies most days, weekly activities and outings, an on-site salon, courtyard walking paths and a facility generator to maintain power during outages. Unfortunately, I am unable to select 0 stars. I would like to warn people of this facility: unless you do not have other options, I highly highly highly recommend staying away. It took me some time to write this up as I was trying to be logical regarding my situation. My grandmother returned to the facility from the hospital the night of 2/23. She was admitted after desatting at the facility prior and noted to have extremely low oxygen. Even upon returning, I received a call from one of the NPs in the evening reporting how in pain she was and the goals to transition to hospice care. We…
Unfortunately, I am unable to select 0 stars. I would like to warn people of this facility: unless you do not have other options, I highly highly highly recommend staying away. It took me some time to write this up as I was trying to be logical regarding my situation. My grandmother returned to the facility from the hospital the night of 2/23. She was admitted after desatting at the facility prior and noted to have extremely low oxygen. Even upon returning, I received a call from one of the NPs in the evening reporting how in pain she was and the goals to transition to hospice care. We specifically talked about sending her back to the hospital if her condition worsened. On the morning of 2/24, there was only one documentation of oxygen check at 7:30AM per the manager I spoke with later that day. They did not document any additional checks despite my grandma being recently discharged and needing use of BiPaP. My grandmother was noted to pass away at 1:14PM. However, there was no documentation of anyone checking on her in between the hours of 7:30 til pronounced death. There is severe neglect of monitoring and care provided by the facility. In addition, upon informing the family of the death, they failed to let them know the facility already contacted the medical examiner's office. Other family members were unable to see her before she was placed at the medical facility. The lack of care and due diligence is incredibly disheartening for a place that is supposed to care for the elderly at their weakest. Even earlier in the stay, the facility never seemed to be aware of my grandma's situation. There were various times where dialysis was cancelled due to holiday/it was rescheduled and we were never informed. My grandmother always seemed to be in the same position in bed, despite them telling us they spend time rotating her (our family visits, every day, multiple times daily). Even during hospitalizations, the nurses were not surprised that my grandmother had sores when we mentioned the facility she resided at. We had no choice, as Paradigm is one of the few locations that allow dialysis in house and my grandmother was too frail for transport. That being said, please pick another facility if you have a choice.
My mother was recently a Rehab patient at Paradigm Katy and I would like to recommend this facility. The clinical staff was caring, attentive and personable. The administrative staff spent a great deal of time with my family sorting thru the paperwork and they were so patient and helpful. I was impressed with the genuine concern over the well being and recovery of my mom.
The nursing home is clean, doesn't smell too bad. There's a lot of staff there working with all the patients. The receptionist is super sweet. Just needs a little update on the rooms. Like new window blinds and chairs in every rooms per bed for visitors to sit on. Especially in the bedridden patients rooms. I forgot to mention that this is now called "Paradigm at Katy".
The staff at this facility are amazing and I especially love the ladies in the business office. Tracie and Mekesha are always available, helpful and dependable.
Paradigm at Katy consistently demonstrates exceptional hospitality. Every staff member — from the front desk to the nurses and kitchen staff — has been kind, attentive, and accommodating. A special thank you to the head of kitchen Mr. C for effortlessly ensuring everyone was well taken care of during meals.
