My wife was here for 20 days and it was a terrible experience. Her medication was always late which with anti seizure medication being late is unacceptable. The place was unsanitary, there was human fecis in the sink of the bathroom near the nurses station sink for 3 days before anyone bothered to clean it up. The water they provided patients came from igloo coolers that you would put can drinks in that were filled with water and ice floating around and they would scoop peoples cups in them and patients had access to do the same with no sanitation before dipping into the shared water source.…

The water they provided patients came from igloo coolers that you would put can drinks in that were filled with water and ice floating around and they would scoop peoples cups in them and patients had access to do the same with no sanitation before dipping into the shared water source. I brought bottles of water the whole time she was a patient. The nurse call would be going off constantly and the staff didn’t seem bothered by it.

A family member was brought here after an extended hospital stay and left in much worse condition. They potentially could have died, given how horrible this place is, if family had not been there every day. I don't say this lightly, but I believe the nursing staff here is genuinely evil. The manager, Ruth, was horrible, hateful, and lazy. The staff were nice when I was around, but my family member told me they would insult and yell at her when no family members were present, and they refused to answer call lights, that part I can not personally verify but here are things I personally saw…

The staff were nice when I was around, but my family member told me they would insult and yell at her when no family members were present, and they refused to answer call lights, that part I can not personally verify but here are things I personally saw while there, when they were on their "best behavior": 1.They ignored the call light for two and a half hours, even after I went to the nurse's station to ask for help. My family member was left in a wheelchair in the middle of the room. She had zero mobility and could not get into bed on her own. 2.It took over an hour and a half to change her after she went number two. The nurse said she would be right back, but it turned out the shift change was in 30 minutes, and she didn't feel like doing it, so she just stalled and went home. She didn't even tell the nurse she handed off to. When my family member finally did get changed, they left the wipes and other supplies on her bed, and I had to dispose of them. 3.A tech brought her food, set it on a tray about three feet from the bed, and left. My family member said they did this all the time. She could not sit up on her own or reach it. The tech would just come back an hour later and take it away, ignoring her when she said she couldn't reach it. 4.They lost one of her antibiotics. A nurse dropped it on her bed, looked for it for a few seconds, then left without saying anything. After an hour, I had to go find her and ask if they were getting a replacement, and she said no. 5. They stole from my family member on three different occasions. First, all her clothes "went missing in the laundry room." They told us the clothes were gone and there was nothing they could do. We had to keep escalating it until the clothes were "found in the corner," and when we asked what that even meant, they did not explain. They also stole her toiletries case with all her makeup in it, which once again was "found" in an office on the other end of the floor after we started getting upset. Lastly, they took the cash out of her purse. When I left in the evening she had cash in her purse, when I came back the next day it was gone. She told us a nurse came in and went through her things during the night. As bad as all of this is, the worst part is that they almost killed her. She had a UTI and a catheter when she arrived. They refused to clean it regularly. We specifically asked them to clean it more often, and Ruth said it isn't standard to clean it every day (this is like very basic nursing that it should be cleaned at very least daily) and they would only do it every few days we believe they went much longer then that. Her UTI got much worse because they didn't give her medication and refused to clean her, until we took her to the ER, where she had to stay for a week on IV antibiotics because the infection had moved up to her kidneys. While we waited for transport to the hospital, we asked Ruth if they could fix the catheter, because it was half out and she was leaking all over herself. Ruth said something to the effect of, "Why should we? She's going to the hospital anyway." and they did not fix it. I have never met a more hateful and lazy group of people. My family member came here for rehab and left in far worse condition than when she arrived.

My 93 yr old Aunt required 2 weeks of 24 care and rehab after a bad fall. The Forth Worth Transitional Care Center took great care of her. She loved the rooms and the staff. They got her up and moving. Thank you so much for the great care you gave my Aunt.

I would strongly encourage families to do thorough research before placing a loved one here. From what I’ve heard, it raised serious concerns about the level of care, staffing, and overall attention given to residents. There appears to be a pattern where negative reviews are quickly followed by multiple positive ones, which makes it difficult to get a clear, honest picture of what’s actually happening inside the facility. At a minimum, I would recommend asking detailed questions about staffing ratios, patient supervision, and incident reporting. These are vulnerable individuals who deserve consistent, attentive care, and I did not feel confident that standard was being met.

Fake reviews. A few days after I posted my 1-star review about my mom’s experience, there were suddenly several 5-star reviews all posted on the same day. One of them was even from a nurse who took care of my mom while she was there. Instead of addressing the very real issues experienced, they are trying to bury negative feedback by flooding the page with positive fake reviews.

My loved one enjoyed their stay and I would recommend them to friends and family !

My dad stayed here for care and the experience for him was amazing, would definitely recommend !!

My loved enjoyed their stay and I would definitely recommend this facility to other family and friends!
