The Right to Individualized Care & Autonomy ​Federal Law: 42 C.F.R. § 483.10(f) / Nursing Home Reform Act of 1987 ​The Violation: Facilities cannot impose sweeping restrictions on all residents equally. Every resident has the right to self-determination, personal routines, and individualized care plans. Blanket bans on specific personal items, dietary choices, or daily schedules ignore individualized assessments and violate resident autonomy. ​2. Illegal Restrictions on Visitation & Advocacy ​Federal Law: 42 C.F.R. § 483.10(f)(4) ​Texas Law: Texas Human Resources Code § 102.003(f) ​The Violation: Facilities sometimes issue blanket bans restricting specific visitors, family members, or advocates under the pretense of "protecting the facility" or avoiding conflict. Restricting access is unlawful unless a visitor is actively disrupting the care or safety of other residents. Facilities cannot use blanket visitation bans to silence family advocates or conceal facility conditions. ​3. Freedom from Unlawful Restraints ​Federal Law: 42 C.F.R. § 483.24(a)(2) ​The Violation: Imposing blanket policies that use physical or chemical restraints (such as routinely administering sedatives, locking doors to whole units without individual necessity, or using bed rails universally) is illegal. Restraints can only be applied with a specific doctor’s order, documented medical necessity, and informed consent. ​4. Do not send your parents/ any loved ones here!!!!! If I could rate it 0 I would. My mother went here strictly for rehab after a long hospital stay. Within 1 day of being here they took her off all her everyday meds including her heart meds and blood thinners. They called us telling us she had 2-3 days to live ( after 1 day of being there!!!) I went right up there and got my mother out. I hope everyone reads these reviews before sending a loved one here.

I agree with my brother time to clean this place up. I find it to be understaffed. And the are pack in there. . There has to be complaint about this. They have no firewall in line. They say my dad has bad memory. Think not he know and remembers everything.. I do not want to hear you excuses have gone on long enough. Wonder how many health code violation you have.. Make me wonder how many fire code violate. Put pation behind lock doors being treat like prisonor. This has to be health code as well fire code violation. They can't get fire department in room if need to get out. Quit with the make…

Make me wonder how many fire code violate. Put pation behind lock doors being treat like prisonor. This has to be health code as well fire code violation. They can't get fire department in room if need to get out. Quit with the make up excuse quit treating people like sardines. And hire people to clean the rooms. I can understand smell but it should not be that strong in place. I bet they have no put fits drill in place which it required by law now . My dad maybe 78 but he not stupid. . I think the state need to be notify of this place. How can you have fire drill when under staff and door are locked. . Make you wonder how many fire codes law being broken here. This is not nursing home it rehab cent be treated as such. They should be rehab and transported else. Where. When you call and no answer serval times to check on dad after few days . I get with brother to drive 2 hours to see why .. please if you family needs help go else where. The clean dinning area and lobby just a front. .

Trying to call and check on my dad and it rings then hang up .... now I am making the drive up there. This is so wrong I cant get ahold of no one to check on him .... so I showed up and his room looked like this behind his TV and windowsill as you can see this place was just plan nasty . Your housekeeping supervisor is there just to make a pay check should being audit the rooms to make sure that the housekeeping is doing there job. Makes me think they just come in a pull trash and tiddy up the bathroom. You need to be me respectful for your patience not just get payments of them . Can't see that the windowsill has spider webs on it

My mother in law was there a month for rehab. Her care was exceptional. She was bedridden and suffers from incontinence and the team always made sure she was dry and clean. They were super responsive to our questions and requests and always had time to discuss her care. I highly recommend this place!

We have visited my brother in law several times. My wife is in contact with thise who are in charge and have a great relationship. Although there are minor bumps in the road, they communicate as rather should and I believe they really are about their residents. Like I said, communication is the key and I hope those who had less than they expected to take a look at the overall picture. 5 Star service

Avir at Walnut Springs is an excellent facility with lots of warmth!

Recently removed my MIL from here. She had been here for the past 4 years. We finally had enough of their excuses. If your loved one is there - please take photo’s with your phone and document or record all phone conversation’s for proof of mismanagement. MIL had a stroke and is a Dementia patient. One day after lunch she must have wrapped her bottom set of false teeth up in a napkin after lunch on her tray. Well they got thrown away. We asked them about getting her a replacement since it was on their watch. And we were told that upper management would not pay to get them replaced and they would adjust her to a soft food diet. Absolutely no responsibility or accountability. She had Covid 3 times and urinary tract infections way too many times over the years because she was left in her diaper’s too long. Especially around holiday’s when they were short staffed. When you call over there they do not even have an answering phone to leave a message. Her room did not have a functioning bathroom. We furnished the curtains and TV for her room. Her clothes closet was always a wreck. Doctor for the facility would only come thru about once every 6 months in person. His assistant would be the main contact and he would prescribe meds and treatment per consults with his assistant. Experienced problems in the scheduling of doctor’s office visit’s in which when my wife got to the doctor’s office at the same time as her mom was transported - - it was - - oh we don’t take patients with that kind of insurance. Shouldn’t the coordinator have confirmed everything the day before ? We started keeping a log this last year and taking pictures. There are a whole lot more things I would like to point out that need correction. I could write a small book about it all. There were a few individual’s that work there that really cared and to them we say thank you. There is a high employee turnover and a lowered occupancy at this facility which was just recently bought out. Go figure.
