Ask about this communityOnpointe Transitional Care at Texas Health Presbyt in Dallas, Texas. This short-term skilled nursing center on the Texas Health Presbyterian campus provides post-hospital rehabilitation and medical oversight, with 24-hour licensed nursing coverage, daily physician or nurse-practitioner rounding, and on-site services such as IV therapy, wound care and respiratory support. Physical, occupational and speech therapy are offered seven days a week in a dedicated gym that includes equipment for gait and ADL practice.
Rooms are available as private or semi-private suites with adjustable beds, TVs, Wi-Fi and accessible bathrooms; many suites were renovated and some include mini-refrigerators. The center sits directly on the hospital campus with an enclosed walkway for access to radiology, pharmacy and emergency care. Dining follows dietitian-guided menus with room service as needed, and the community schedules daily life-enrichment activities and family lounges.

It's was great Mr. R. Sanchez was very motivating each morning his kind words helped me start my day. Brittany the wound care nurse was wonderful she made my wounds feel better and she always had a kind words and made me feel comfortable. Jorge he started a news 📰 letter and they are so inspirational if you felt down somehow the news letter would fulfill your mood. Kristie she is the head nurse and was so excited and she could put a smile on your face that was real. And she made sure that you were comfortable.
This is located on the 5th floor of the Main Building of Texas Presbyterian Hospital. I had the opportunity of spending time there on two separate occasions while caring for a family member. Compared to the other floors of the building, this floor seems like a ship with a dead engine, floating aimlessly in the ocean, and staffed by mannequins that move. The staff seriously need a refresher course in civic interpersonal communications. The verbal responses are in monosyllables and smiles seem to be a taboo. And that goes for the receptionists as well. Response times of the nursing staff here…

And that goes for the receptionists as well. Response times of the nursing staff here need to be multiplied five times as anywhere else, to be of any value. Even after calling for a nurse's attention/assistance on the SonifiHealth pillow speaker remote, a patient mostly needs to wait an hour or so to get a response. And by then, the patient's situation gets worse. The alarm keeps buzzing at the nurses' station, but no one seems to care. I had the opportunity of talking to an elderly patient there, who had a surgery of his ankle, and was wheelchair bound. He seemed equally disappointed with the level of service he was receiving. To quote him: "I press the button for a nurse's attention, but an hour or so goes by before anyone comes" and "No one here smiles." But all that does not take away from the fact that a handful few of the staff are indeed helpful, compassionate - and they do manage a smile or two along the way. But, unfortunately, that group is a pathetic minority. And if the excuse is that the place is under-staffed, that ought to be looked into. The only bright spot in this place are the physiotherapists, who are always helpful, communicative, responsive - and who also smile a lot. Overall, I'll have to admit that I was deeply disappointed with the quality of service and patient care at this place.
Can't recommend this place. The PT is good there but if you had surgery and need lots of care, it's not good. The doctor is not good. He gave me wrong treatment because he didn't talk to me and I spent the rest of time there trying to undo this wrong treatment. Never saw him for discharge so left without knowing what I should do. Some good caregivers work there and some not, like everywhere. But they do have good PT if that's what you need most.
I dont know anyone else's experience but for me Pure Health is really taking care of my glammah... D.O.N Kristi I appreciate you being straight forth, clear with explaining everything to me without medical terminology, just FACTS! My spirit picked up on stress in tiredness in your eyes. Nursing is a hard job and requires much from you mentally and physically. THANK YOU FOR WHAT YOU DO! Im praying that you and your team will get the peace and strength you need. We need you all. You offered me resources that I REALLY need prior to discharge. Before i left i was not confused. Thank you KRISTI DOR Ms Anita same as Kristi, she answered my questions and she also was clear, she has a kind, calm demeanor that left me comfortable knowing that my glam-ma is in good hands. I pray for you too. THANK YOU! The Discharge Planner,Jesus, Explained the plan for discharge clearly. Offered his help if I need it. He is very sweet and helpful. THANK YOU! My glam-ma is getting stronger. I was there when her tech came to clean her and she didnt know I was standing by the door and she was very gental and careful with her. I didnt get her name tho. I rechecked my glam-mah to make sure she was wiped and cleaned well and she was.. THANK YOU TOO, LADY BUG. You are a blessing My Glam-mah said everyone is nice to her. From ICU to the rehab. Im very comfortable wuth her being here. You all are secret agent Angels to my Glam-ma... GOD BLESS YOU ALL! THANK YOU!
My mother was transferred to this facility from Presbyterian based on assurances that they were fully equipped to handle her medical needs. That was not true. Upon arrival, they did not even have the oxygen she required—we had to provide it ourselves. During the 6–7 days she was there, she was not bathed or assisted with basic hygiene. The food provided was consistently cold, hard, and unacceptable. She received physical therapy only 2–3 times at most. Staff repeatedly failed to respond when she pressed the call button for help. Her condition declined significantly under their care, and she was ultimately sent back to the ER with an infection and salmonella poisoning. She later passed away. This level of care is unacceptable. I would not recommend this facility to anyone seeking proper care for a loved one.
My mother, age 92, stayed at PureHealth Dallas for approximately 3-weeks throughout March and April 2026. The services she and our family received were most professional and welcoming. On behalf of our entire family, we formally thank our care team comprising: Jorge, Kristi, Jesus, Anita, Katherina, Robert, Diana, Fatuma, Taminda, and Rodrigo. Everyone is simply, Awesome!! Don’t bring anyone to Pure Health!!! My husband stayed here for 13 days. Torture. Terrible food… so tasteless he would not eat it. Tried to give him wrong meds on more than one occasion. Received one sponge bath the whole time we were there. Tv didn’t work for the last 3 days we were there and it was reported. A couple of the staff are warm and caring, The rest are very unconcerned. I left a note for the charge nurse to call me. She never called. The leadership people will tell you that they will get back to you with answers to questions. They never return. I heard more than once about their “short staffing.” I could go on and on and on with a list of problems. I would have packed up and left but my husband has Alzheimer’s and can’t be left alone so I couldn’t move our things out. Also, I had to wait for his memory care room to be ready. You are totally on your own. Expect no help. JW
Do not stay here! My friend was here for recovery from an infection after knee surgery. She had trouble getting her pain meds, her pick line clogged twice because this facility doesn’t carry heparin. There was very little communication. Small things like she asked for a towel to take a shower. Almost 30 days and they never brought a towel. PT folks were good but there must be better facilities.
