After visiting a friend here repeatedly, I can't let this go and hope that other famillies really do your homework about this facility. The conditions are deplorable here!!!! The place is filthy, patients are ingored for hours on end, only to leave them helpless and sitting/lying in very unsanitary and inhumane conditions. My heart aches for these precious elderly people who are unable to do for themselves, no family around, and the people left to care for them here, don't feel like doing their job. The administrator is always berating the employees in front of the patients......very…
After visiting a friend here repeatedly, I can't let this go and hope that other famillies really do your homework about this facility. The conditions are deplorable here!!!! The place is filthy, patients are ingored for hours on end, only to leave them helpless and sitting/lying in very unsanitary and inhumane conditions. My heart aches for these precious elderly people who are unable to do for themselves, no family around, and the people left to care for them here, don't feel like doing their job. The administrator is always berating the employees in front of the patients......very unprofessional!!! We are in the process of looking for a new facility. Please, please, please........check on your loved ones........talk to them, lay eyes on them, show up unannounced, but just dont dump them in a home.
I love the new maintenance team at the building. Seeing someone so young working as hard as he does shocks me every time I come by. He’s one of the hardest workers I’ve seen, and it really shows in how well the property is being maintained. I also want to give a shoutout to the new DON. You can tell the new team is coming together, and the improvements are noticeable. The only complaint my mom has is that her clothes are not always coming back to her after being washed. Building always looks nice and The Nursing care is also approving there. Wright..
This place failed my mom. We brought her here to continue her rehabilitation and recovery after a stroke, and after two months she ended up having another stroke that has left her in a worst state. It turns out that in the two months she was there, they never administered her thyroid medicine, and that was one of the things that led to her having another stroke. As if that wasn’t bad enough, the staff member that was with her at the time of the stroke, didn’t even realize she was having one. I know this because she called me on my moms iPad so I could speak to her, only for me to see that my…

I know this because she called me on my moms iPad so I could speak to her, only for me to see that my mom had gone mute and was unable to speak - I then told her to immediately call 911 because she was clearly having a stroke. Who knows how much more time would have been wasted if I hadn't answered that call?
I am the administrator of Southern Oaks and I can attest that nothing stated by Ms. Harris is true. If Ms. Harris wants to know the truth, she is welcomed to visit facility of call to speak to me. Our ombudsman can also share update on her mom's situation. We do not discuss resident's information hence why I cannot share details of what is the truth. The facility's CMS star rating is testament of our level of care and facility compliance with state and federal regulations. Every facility has issues, we certainly have our, but nothing alleged by Ms. Harris is true. Her allegations were proven to be false and all were unsubstantiated.
My mother has been at the facility for almost two years. She has been trying to transfer to another facility close to family but the facility refuses to give her a clinical review in order for her to leave. Instead - they ignore her, have staff stealing her nice clothing then wearing it in front of her, and refuse to change her on the weekends. At one time a staff who is no longer there, somehow had her Medicaid cut off - so that she could not transfer. My mother has since gotten her Medicaid reinstated when the facility saw that she could not pay rent without it. I don’t understand how this facility remains open even though they have had several state write-ups of serious issues? If I could personally move my mother to a better facility I would. Due to her eligibility for Medicaid and Medicare services and her refusal to make anyone her proxy, this is what many have to deal with.

Moved my dad in the facility, August 4th from another facility, he had not took his medicine or had anything to drink or eat for over 2 weeks no fault of the facility. It was just he woke up one morning, wirh dementia. And he decided he didn't want to eat of drink anymore. He walked away from the facility, so they weren't equip, to take care of him, you know, because he was walking away. So we moved in here, he's doing so so much better. The physical therapist, his speech therapist, he's walking around, talking noise, he has his strength back and he's eating. I mean, he's eating and I am so thankful for this facility here and the staff.
