It was wonderful to see the guests so engaged, happy, and motivated as they worked hard to reach their goals. The Administrator, Kelsey, is hands-on, supportive, and clearly committed to creating an environment where both guests and staff can succeed. Amazing team! The Director of Rehabilitation, Kimberly, is incredibly passionate and leads a long-tenured therapy team with such dedication and heart. The Administrator, Kelsey, is hands-on, supportive, and clearly committed to creating an environment where both guests and staff can succeed. If you or a loved one is focused on reaching therapy goals and getting back to doing the things you love, this is the place! They truly set everyone up for success with a very goal-oriented and even provide an outpatient therapy program. It’s clear that this team cares about outcomes, but even more importantly, they care about the people they serve. ❤️
My mother was a resident at Mallard Creek from April through June 2026, including during the change in ownership. Our experience was marked by poor communication and a lack of attention to her complex medical needs, including her diabetes. As a result, our family ultimately lost confidence in the facility’s ability to safely manage her care. Based on our experience, I would not recommend this facility for a loved one with complex or high medical needs.
My nephew Carlos is very happy at Mallard Creek, he was at several different places that were a nightmare. We are blessed that Gina reached out to me and we immediately moved him. It’s the best move ever for my nephew. He’s loved by the entire staff there. It’s very clean and we don’t have to worry about him.

I came to Mallard Creek needing two therapists just to help me get out of bed. Thanks to the therapy team, I left walking on my own. Their skill, encouragement, and dedication made all the difference. Thank you therapy team. .
Very clean, good caring staff
The building is extremely clean and the staff are wonderful. Mallard Creek is an excellent choice for short term rehab or long term care.
The Receiptionist was amazing! She Answered all our questions with grace , Very sweet young lady!
Ashley is really one of the best housekeepers I've seen in a long time! She really goes above and beyond for her job to help others I truly appreciate her an her hard work.
