4.3 · 93 reviewsFacility has many issues. Meds left ungiven , beds left dirty , facilities management seen yell in g and cussing on my visit. I wouldn’t put my parents in this facility after what we gone thru. Check your parents if they are there you’ll see
Crazy thing. I am a licensed nursing home administrator myself. I was visiting my son in Henderson, Tx. I fell down while getting a bottle of water out of his refrigerator and fractured my left hip. I ended up choosing Autumn Leaves for recovery from hip surgery. Keep in mind I am a licensed nursing and rehab administrator myself, and I can honestly say without a doubt, my decision to admit to Autumn Leaves for rehab was the best decision I could have possibly made! Crazy thing. I am a licensed nursing home administrator myself. I was visiting my son in Henderson, Tx. I fell down while getting a bottle of water out of his refrigerator and fractured my left hip. I ended up choosing Autumn Leaves for recovery from hip surgery. Keep in mind I am a licensed nursing and rehab administrator myself, and I can honestly say without a doubt, my decision to admit to Autumn Leaves for rehab was the best decision I could have possibly made! Autumn Leaves rocks! Their therapists are terrific! A special thanks to Summer, Wessly, Lance, and Karen. They got me back on my feet! The Dietary Department always had a substitute menu incase you did not care for what was on the main menu that day. I recommend the fruit plate! Yum yum. My nurses and nursing assistants were top-notch! Thank you all! It was clean and my laundry was done just right. Buddy the Business Manager was professional in every aspect! This place is disgusting they dropped my nana on her face, and did nothing about it! They have not called or talked to the family as of yet! Has not did anything to show that they care for there residents. I’m truly saddened to right this but something has to be done. The elderly people need to be taking care way better than they are!! Do not I mean do not send your love ones here!!!

Great staff, friendly.
My sister and I drove an hour to come visit our 97 year old aunt. She was sitting up in a wheelchair for the first day since she broke her back and came to tour facility. She was clean, dressed and in good spirits although she didn’t know we were coming. The staff was very nice. You served her a beautiful delicious looking nutritious meal while we were there. The building smelled fresh at 5:00 in the evening and everything was neat and clean. We both were greatly impressed with the place and felt good about our aunt being there. Keep up the good work!
