At that the last two stays for my grandmother were exceptional. the staff was very friendly and catered to her needs and the family. The facility was always clean, stafff friendly and the PT/OT staff were amazing. They were able to get her in top shape to go home. They were able to put her in rooms close to the gym and in a quiet hall, were she could walk and be in peace. There was no issues at the time we used them. But now they are under new management, ive read some reviews and because of that it makes deciding to bring her back here…

We have used this facility, when it was Founder's Plaza. At that the last two stays for my grandmother were exceptional. the staff was very friendly and catered to her needs and the family. The facility was always clean, stafff friendly and the PT/OT staff were amazing. They were able to get her in top shape to go home. They were able to put her in rooms close to the gym and in a quiet hall, were she could walk and be in peace. There was no issues at the time we used them. But now they are under new management, ive read some reviews and because of that it makes deciding to bring her back here easier. Hopefully it works out...
Wylie Oaks, previously known as Founder’s Plaza, has been extraordinary. The therapy department is “Top-notch.” They assisted my mother after suffering a hip fracture. They assisted her get home as soon as possible and were even able to follow her with their amazing out-patient services. Thanks for taking care of my mom like she’s family. Completely recommend!

Great facility and staff!!
What a breath of fresh air! The new team and ownership here has really proven that they care. A fantastic therapy team with great nursing and overwhelming compassion and care.
This place is amazing!! Such a friendly staff, open area doesn't feel so confined. It has 2 gyms for the residents one good size gym at the front and a large newly equipped gym at the end of one of the halls. The therapist are so kind and warm hearted they really care about each other and their residents.
Nice facility with some single occupancy rooms. New admin Richard is friendly and responsive.
